Vitamin D, B12 and folate: reading your results

Vitamin D, B12 and folate are three of the most commonly tested micronutrients in the UK - and three of the most commonly low. They turn up together on a lot of blood panels because deficiencies are widespread, the symptoms overlap, and two of them (B12 and folate) are closely linked in the body. When one comes back flagged, it is natural to wonder what the number actually means.

This guide explains what each vitamin does, the typical UK reference ranges and units you will see on a report, why low results are so common here, and how B12 and folate relate to one another. What each vitamin does

Vitamin D helps your body absorb calcium and supports bone, muscle and immune function. Unusually for a vitamin, most of it is made in your skin when it is exposed to sunlight, rather than coming from food - which is exactly why it is such a problem at UK latitudes.

Vitamin B12 and folate (vitamin B9) work as a pair. Both are needed to make healthy red blood cells and to support nerve function, and both feed into the same biochemical pathways. A shortfall in either can show up as tiredness, or as larger-than-normal red blood cells on a full blood count.

Typical UK reference ranges and units

Ranges and units vary between laboratories, so always read your result against the range printed on your own report. As a rough UK guide, vitamin D is usually reported in nmol/L with widely used bandings:

  • Vitamin D - deficient: below 25 nmol/L
  • Vitamin D - insufficient: roughly 25–50 nmol/L
  • Vitamin D - sufficient: above 50 nmol/L (many labs cite ~50–125 nmol/L as the target band)
  • Vitamin B12 (serum): roughly 180–900 ng/L (a value at the low end may prompt an active-B12 or MMA check)
  • Folate (serum): roughly above 3–4 µg/L, with values below that flagged low

Why deficiencies are common in the UK

Vitamin D is the standout. From about October to March, the sunlight in the UK is too weak for the skin to make meaningful amounts, so stores built up over summer have to last the winter. Public health advice in the UK is that many people may benefit from considering a vitamin D supplement over the darker months - a useful illustration of how routine low results are here.

B12 and folate have different drivers. B12 comes almost entirely from animal foods, so people eating little or no meat, fish, eggs or dairy can fall short without a supplement or fortified foods. Folate comes from leafy greens, pulses and fortified products, and intake is easy to let slip. Absorption matters too: some medicines and gut conditions reduce how well B12 or folate are taken up, which is one reason a low result is not automatically about diet.

How B12 and folate interrelate

Because B12 and folate share the same pathways, they can mask each other. Folate supplementation, for instance, can improve the blood-count picture while an underlying B12 shortfall continues - which is why doctors are careful to assess the two together rather than topping up one in isolation.

This is also why a single number rarely settles the question. A borderline B12 might be read alongside an active-B12 (holotranscobalamin) or methylmalonic acid test, and a low folate is more meaningful when seen next to your red-cell indices on a full blood count. Looking at the markers as a set, and tracking them over repeat tests, gives a far cleaner picture than reacting to one snapshot.

Dietary sources and when supplements are a doctor conversation

If you want to support your intake through food, oily fish, eggs and fortified spreads contribute some vitamin D; meat, fish, eggs and dairy supply B12; and leafy greens, pulses, beans and fortified breakfast cereals provide folate. For most people a varied diet plus sensible sunlight does much of the work, with vitamin D being the common winter exception.

Supplementation is where it pays to involve a clinician rather than self-prescribe. The right dose depends on how low you are and why, high-dose vitamin D needs care, and a low B12 in particular should be investigated for cause and sometimes treated with injections rather than tablets. Use your results to start an informed conversation with your doctor, not to diagnose yourself.
